Legacy Care is seeking an energetic, compassionate, and results-driven Community Liaison to build lasting relationships with hospitals, physicians, assisted living communities, skilled nursing facilities, senior organizations, and healthcare professionals throughout North San Diego County.
Our mission is simple: Help older adults with chronic illnesses remain safely and comfortably at home while giving families peace of mind.
What You'll Do
- Develop referral partnerships with healthcare providers and senior living communities.
- Schedule and conduct face-to-face visits with referral sources every week.
- Represent Legacy Care at networking events, health fairs, and community meetings.
- Educate healthcare professionals about our concierge-level home care services.
- Follow up consistently to build trust and generate referrals.
- Meet weekly outreach and referral goals while maintaining exceptional professionalism.
What We're Looking For
- Experience in healthcare marketing, home care, hospice, home health, senior living, or medical sales preferred.
- Outstanding communication and relationship-building skills.
- Self-motivated with the ability to work independently.
- Professional appearance and positive attitude.
- Reliable transportation and willingness to travel throughout North San Diego County.
- Passion for serving seniors and their families.

We specialize in care management, care navigation, transitional care management and more for the elderly and retired executives. We provide non-medical assisted living service to our clients wherever they call home.
